LifeLong Medical Care is looking for a Psychosocial Services Manager at our TRUST Health Center in Oakland. The Psychosocial Services Manager provides strategic direction for developing and implementing non-clinical services that support patients to achieve their health and wellness goals. The position directs case management, housing navigation, and health coaching services. Responsibilities include oversight of related policies and procedures, operations, care models, staff recruitment and training, supervision, budgeting, documentation and reporting, quality improvement, and collaboration with stakeholders and funders. The Psychosocial Services Manager is member of the site leadership team and collaborates with other LifeLong departments as well as public and community- based agencies to improve access and quality of care.

Benefits

Responsibilities

Hires, trains, supervises, and evaluates non-clinical psychosocial support staff including and Housing Case Manager
Provides ongoing technical and subject matter support to case managers
Offers individualized coaching, works with HR to address any employee performance concerns, and implements performance improvement plans as needed
Collaborates with ABHD, Center Director, and AMD to develop a clinic services model that promotes patient centered, integrated, team-based care
Plans psychosocial service programs, develops systems, and establishes protocols to meet requirements of funders and to achieve quality improvement and assurance goals, including productivity and caseload standards
Helps plan, facilitates and participates in clinic, department and agency meetings
Collaborates across LifeLong homeless services programs to promote care coordination and seamless system of care
Actively participates in community collaboratives and represents LifeLong at external meetings and with funders
Participates in planning and implementation of data collection and quality improvement activities related to psychosocial services and integrated care
Collaborates with leadership team to develop and monitor the site and grant funding
Promotes diversity, equity, inclusion and belonging in support of patients and staff
Performs other duties as assigned

Qualifications

Education & Experience

Bachelor’s degree required or 3 years minimum supervisorial experience, master’s degree preferred
At least 3 years experience providing case management or related services to disadvantaged populations
Experience in a publicly funded mental health program, community health center, and/or social service agency serving complex patients with histories of homelessness and behavioral health conditions
Program planning, management, budgeting

Knowledge Skills & Abilities

Experience with case management, individual and group models of care, crisis intervention and staff education and training
Strong commitment to working with a low-income, ethnically diverse patient population
Strong knowledge of mental health, substance use and psychosocial issues
Strong organizational, administrative and problem-solving skills and ability to be adaptive to change
Knowledge of current developments and evidence-based practices in harm reduction and trauma informed care
Knowledge of coaching techniques and human behavior change
Excellent interpersonal, verbal, and written skills
Must be proficient in Word, Excel, and basic office IT systems
