Axis Community Health has served the Pleasanton, Livermore, and Dublin area since 1972. Our services include medical care, mental health counseling, behavioral health, and WIC nutrition services for Tri- Valley residents.

ESSENTIAL DUTIES/RESPONSIBILITIES:
• Provide direct primary care medical services to include history, diagnosis, treatment, referral and follow-up of patients.
• Provide all services in accordance with accepted medical standards, organizational policies, contractual requirements, and in a manner that is responsive to the needs of all patients.
• Maintain a high level of awareness of evolving medical practices and work in collaboration with the Chief Medical Officer to develop and implement new practices and protocols as appropriate.
• Participate in the 24-hour call schedule when required and provide appropriate medical triage according to established protocols.
• Participate in quality assurance, peer review and utilization review activities as assigned.
• Perform other duties as assigned.

REQUIRED COMPETENCIES:
• Expertise in field of board certification.
• Ability to keep accurate, clear and legible records. Ability to use Electronic Health Records
• Bilingual English/Spanish and/or other languages highly desirable.
• Ability to interact effectively and in a supportive manner with persons of all backgrounds.

QUALIFICATIONS:
• M.D. or D.O degree from a recognized University, one to two years experience working in primary care clinic setting.
• Current valid California medical license, DEA, CPR/AED.
• Board certification required.

Physical Demands and Working Conditions:
Work is normally performed in a typical office work environment. While performing the responsibilities of this position, the individual is required to have ambulatory skills sufficient to visit various parts of the organization. Requires the ability to reach with hands and arms, climb stairs and occasionally lift and/or move up to 30lbs. Requires sitting and standing for extensive periods of time, operating a computer keyboard, telephone and other equipment for extended periods of time. Travel to other Axis health centers and other local travel will be required. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
