IMT Associates, a private professional fiduciary practice, is seeking a case manager to join our
team. Our practice focuses on the administration of trusts, estates, conservatorships and case
management of elderly, mentally ill and disabled clients. Our ideal candidate is someone
pursuing a career in counseling or social work. The position is supervised by a licensed
Marriage & Family Therapist and may count towards hours required for licensing. This
individual must enjoy working with our client population and do well in challenging situations.
This person must also be dependable, honest, compassionate, professional, punctual, detail
oriented, highly organized, and an excellent communicator.
Primary Responsibilities:
*Manage caseload of 15-20 clients.
*Conduct frequent client visits at home or in assisted living/skilled nursing facilities.
*Engage with facility staff or in-home support teams to ensure client’s needs are met, attend
quarterly care conferences.
*Manage SSI & Medi-Cal benefits.
*Assess clients’ housing situation and make necessary changes.
*Keep key stakeholders (family, friends) informed of client’s status & changes, encourage social
connections.
*Understand client’s financial situation and manage to established budget.
*Review & authorize expenses incurred on client’s behalf.
*Track and log billable time to appropriate account.
Skills and Qualifications:
*Use of own insured vehicle.
*Available to work weekdays at our office in San Leandro.
*Bachelor’s degree and minimum of 3 years’ experience in case management field.
*Master’s degree preferred.
*Proficiency in Microsoft Word, Excel, and Outlook.
*Knowledge of social security benefits is a plus.
