Overview

Marin Community Clinics is looking for a mission-driven Pediatric Associate Case Manager to join our dynamic, multidisciplinary, Complex Care team to advance our goal of improving health outcomes for individuals with complex medical, behavioral health, and psychosocial needs. Our Complex Care team is committed to increasing health equity among Marin’s most vulnerable patients through person-centered, trauma-informed, case management support.

The LVN is responsible for performing floor procedures and nurse case management functions. The LVN tasks also include facilitating drop-in TB and STD clinics under RN and Physician oversight. Tasks also include follow-up and tracking of HIV/AIDs, TB, HEP C. This nurse will also assist with PrEP navigation. The LVN will perform office duties, such as assessing labs and consulting with RN/provider as needed, answering the Nurse Phone line, and setting up appropriate appointments with patients and outside providers. The LVN also reinforces health education, and other functions to ensure a smoothly functioning clinic.

Responsibilities

50% Floor/Pod Nurse & 50% Infectious Diseases r responsibilities:

Manages and updates patients lists with HIV/AIDs, TB, HEP C.
Assists with drop-In TB and STD clinics as needed.
Follows policies and procedures including universal precautions as defined by OSHA’s guidelines on Bloodborne Pathogens and Hazardous Materials standards.
Maintains patient confidentiality in accordance to department policy and HIPAA guidelines.
Attends and participates in required meetings, training and educational programs.
Performs administrative responsibilities, including checking eligibility, verifying demographics, assisting patients with filling out necessary paperwork, and making up follow-up appointments.
Follows up on phone calls, task and chart reviews in a timely manner as established by department.
Performs floor procedures, including but not limited to; catheterizations, phlebotomy, wound care, EKG, assist providers, and perform procedures accurately and thoroughly per protocol and as demonstrated on the skills assessment.
Administers injectable medications and immunizations following standard procedures and as demonstrated on the skills assessment.
Administers and monitors oxygen and nebulizers per protocol
Reinforces patient education as necessary, including but not limited to: HIV, Hep C, active & latent TB, asthma, diabetes, glucose monitoring, medications, wound care, signs and symptoms to report of disease process. Provides appropriate handouts and documentation.
Assists the nursing team in maintaining crash cart with monthly checks.
Responds to patient requests for lab results, health questions, and questions and concerns regarding disease processes. Seeks additional information and asks for assistance as needed.
Actively assist MAs in maintaining efficient flow rooming patients, completing MA duties, and directing flow as necessary.
Performs all MA duties as necessary to provide breaks or fill-in as needed. Refer to the MA job description -- standards.
Proficiently uses ARIES to document labs, office visits, and other aspects of case management.
Assists with medication refills per protocol. Monitor requests for narcotics and audit charts and patient requests for narcotics to assure adherence to policies.
Be resource for Medical Assistants in decision making and patient safety.
Initiates referrals to public health programs and home-health programs.
Completes all required reporting to the Public Health Department for Infectious Disease patients and participates in case conferences as needed
Manages in-box messages from patients and medical providers.
Coordinates with pharmacies to ensure specialty medications are received by patients in a timely manner to avoid gaps in treatment
Performs continuous patient outreach to ensure Specialty Clinic patients remain engaged in care
Refers patients for insurance screening and assists with completing renewal paperwork for specialty insurance programs (Ryan White and ADAP)
Coordinates with external agencies in providing Ryan White services to HIV patients
Schedules and collect specimen for TB testing, including but not limited to Sputum inductions
Communicates with staff effectively using multiple platforms including but not limited to email, Trillian, phone calls.
Other duties as assigned.

Qualifications

Education and Experience:

High School Diploma or Equivalent (GED) required.
Current California Vocational Nurse License in good standing.
Current CPR/BLS certification.
Experience with EPIC Electronic Health Records preferred.
Minimum of one year working experience in community health outpatient settings/Federally Qualified Health Centers (FQHC) a desired
Prior working experience with HIV patients is highly desired.
Experience working with Ryan White program desired but not required.
Experience with ARIES management system desired but not required

Required Skills and Abilities:

Must be able to work independently with written protocols as well as within a team setting.
Must be sensitive to ethnic and cultural differences and their relevance to health care.
Bilingual English-Spanish is required.
Excellent verbal and written skills.
Ability to follow instructions and meet deadlines..
Must have ability to multitask in fast paced health-care clinic environment.
High attention to detail and ability to document accurately.
Good organizational skills and ability to multitask.
Excellent interpersonal skills and strong critical thinking ability.
Ability to work in fast-paced environment.

Physical Requirements and Working Conditions:

Prolonged periods of typing and sitting at a computer.
Must be able to lift up to 15 pounds at a time.
