Position Description:
The Licensed Vocational Nurse (LVN) or Licensed Psych Tech (LPT) is responsible for providing field-based nursing care, complex medical care management, and at-home care to HIV positive clients enrolled in Intensive Case Management services. The LVN / LPT is the central medical provider on an integrated, mobile team. The LVN / LPT is responsible for supporting health and psychiatric assessments as well as developing and continually updating collaborative client care plans, specifically around medication adherence, care engagement, referrals, and health education. Direct client work and interaction will be done in the field and at the office.

Essential Functions:
• Provides mobile nursing care (under the supervision of the primary care provider and/or director of nursing) and case management (under the supervision of the clinical supervisor) to HIV positive clients enrolled in the Intensive Case Management programs.
• Supports field-based health and psychiatric assessments and monitoring.
• Offers enhanced medication support; this includes dispensing, directly-observed treatment (DOT), and IM Depo injections.
• Coordinates directly with pharmacies to support appropriate medication treatment.
• Provides complex care management such as hospital transition support, medical referrals, and health education.
• Provides service referral, linkage, coordination, and advocacy services.
• Develops and renews clients’ individualized nursing treatment plans.
• Coordinates with clients’ primary care provider on a weekly basis.
• Completes necessary documentation, Unit-of-Service (UOS) reports and treatment plans in a timely manner.
• Participates in weekly case conferences, monthly staff meetings, and in-service trainings.
• Performs other duties and tasks as assigned.

Minimum Qualifications:
• Candidate must be a Licensed Vocational Nurse or Licensed Psych Tech in the state of California
• Knowledge of HIV/AIDS, STIs, substance use, and mental health issues.
• Experience working with LGBTQ populations.
• Familiarity with HIV/AIDS issues in communities of color.
• Experience working in a team-based care environment.
• Knowledge of harm reduction philosophy, client centered counseling, and motivational interviewing techniques.
• Must be adaptive, flexible, and a team player.
• Strong verbal and written proficiency in English.
• Fully vaccinated against COVID-19, complete with booster shot(s) and able to show proof of vaccination
• Up-to-date with vaccinations, especially seasonal flu shot vaccine
• Proof of TB clearance (issued within the past 12 months or less)
